ACEs-LA’s Network of Care initiative (NoC) is a collaborative effort between LA County DHS clinics and community-based organizations (CBOs) throughout LA County to jointly meet the needs of children and families impacted by toxic stress.
The NoC provides an infrastructure that supports screened and unscreened patients and their families and allows clinical care teams and community service providers to become aware of each other’s work. Through a partnership with the One Degree referral platform, our goal is to optimize multidirectional social, mental, developmental, and behavioral health referrals between DHS clinics and CBOs providing multidisciplinary services for children and families with complex needs.
NoC Clinics |
The Network of Care partners with 13 LA County DHS Clinical sites, which screen for ACEs and refer patients to community partners via the One Degree Closed-Loop Referral system. Each clinic has its collection of Network CBOs to address their social needs and gaps. |
NoC CBOs |
Our network comprises of 38 CBO partners throughout LA County, offering various services to address DHS patients’ needs. Patients are referred directly to CBOs via the One Degree Closed-Loop Referral system, enabling open communication and updates between Clinical referrers and CBOs. |
NoC CAB | |
Members: | The CAB is composed of 11 representatives from CBOs who can offer a strategic and comprehensive range of perspectives, experiences, and skills that align with the mission and core values of the NoC. The CAB’s diversity represents all LA County Service Planning Areas (SPAs) and services offered to DHS patients and families.
