Network of Care

Haven Services has been an active participant in the ACEs LA Network of Care, strengthening our capacity to meet families where they are and address the impacts of toxic stress through coordinated, whole-person support. Through our participation, Haven has strengthened referral pathways with DHS clinics, deepened cross-sector collaboration, and expanded access to our financial empowerment, housing stability, and supportive services. Engagement with the Network, including the use of One Degree and partnership with Community Navigators, has helped our team respond more efficiently to client needs while reinforcing our trauma-informed, equity-centered approach to serving the community.

Getting a therapist or mental health provider can be quite confusing and unfortunately for a lot of pediatric clinics (including ours) theres not a clear linear pathway from your doctor to the mental health team. One Degree has helped bridge this gap and provided proactive calls from a fantastic mental health group (there are several) which then contacts the family instead of putting all the onus on the patient. This has made the process of getting patients the help they need so much smoother and patients seem much more satisfied!

My experience working with the Network of Care has reinforced the power of collaboration. Through NOC partnerships, we’ve been able to share resources, coordinate care more effectively, and ensure families receive the support they need when they need it most.

San Fernando Valley Community Mental Health Center is proud to partner with ACEs-LA Network and other community-based organizations to expand ACEs screening and awareness for the families we serve. Together, we share resources, streamline referrals, and strengthen trauma‑informed care for underserved communities. Through supporting the efforts and clinics of LA County Department of Health Services and the ACEs‑LA initiative, we join a countywide effort to build resilience and ensure that children and families impacted by toxic stress receive coordinated, compassionate support. We are honored to be a part of such a wonderful healing community touching the lives of so many here in Los Angeles County.

TASK’s collaboration with NOC partners provides parents with the knowledge, advocacy skills, and resources they need to navigate the special education process successfully. This partnership strengthens parental outreach and ensures families feel supported, rather than left to navigate complex systems alone.
